Claim Procedure
- Entity shall notify LGLP in writing within ten (10)
days of occurrence or knowledge of occurrence of any
accident, occurrence or loss which could reasonably
be assumed would produce a liability claim.
- For accidents or occurrences involving bodily injury,
LGLP should be notified as soon as practical by phone,
followed by the required written notice.
- Claimant shall complete claim form
- Claimant shall file claim form with the governmental
entity in which claim is against.
- The entity shall forward the original notice of claim
or lawsuit to LGLP immediately following
receipt,
designating:
a. the date received;
b. if personally presented, by whom and date served;
c. if by mail, date received;
d. any and all correspondence received from the party
submitting the claim.
- The entity shall also prepare and forward to LGLP a written narrative stating:
a. Time, place and circumstances giving rise to
the claim or occurrence;
b. Identity, address and phone number of persons
involved, including any witnesses;
c. Statements of circumstances from employee(s)
involved in or witness to the accident; and
d. Name, title, address and phone number of entity
representative that will be available for further contact
- LGLP will investigate claim and notify claimant
and entity of its findings.
