It's Renewal Time!
It's time to renew your membership in the Local Government Liability Pool (LGLP). The new and more concise renewal application is available on this website. Please print either the MS Word or Adobe pdf version, complete it, and return it to us via email, fax, or "snail mail". Please refer to the instructions or call us if you have any questions. For members that have payroll expenditures, we need the four (4) quarterly IRS 941 forms for calendar year 2010. The earlier you submit your renewal application, the quicker we can calculate your contribution. Your renewal application is requested by May 1, 2011. Your contribution payment is due July 1, 2011.
